Join our patient-centered healthcare team as the RuralReach Administrator and own the launch, day-to-day performance, and continuous improvement of Well&You’s rural program from the operational side. This is not a routine site-administration job — it’s the chance to be the operational builder behind a new four-clinic network, turning an award and a strategic plan into sites that open on time, earn community trust, deliver reliable access, and operate sustainably across rural Baker and Nassau counties.

Duties/Responsibilities

  • Lead the operational opening and stabilization of four permanent rural clinics, including site selection and lease coordination, buildout and outfitting, licensure and accreditation readiness, equipment, IT and telehealth connectivity, workflow design, and go-live.
  • Run day-to-day rural clinic operations and outreach events, including readiness, opening and closing procedures, staffing coverage, supplies, equipment, patient flow, safety, and rapid resolution of site issues.
  • Coordinate recruiting, onboarding, training, scheduling, and performance support for rural staff; partner with clinical leaders to ensure each clinic day is appropriately staffed, clinically supported, and productive.
  • Coordinate with physicians, APRNs, and clinical teams so patients can access chronic disease management, vascular and limb-salvage care, cardiac monitoring, women’s health, and non-opioid pain care, with timely follow-up and clinically appropriate connection to hub-based specialty services.
  • Build durable relationships with rural primary care practices, rural health clinics, FQHCs, critical access hospitals, EMS, county health departments, senior centers, employers, and community organizations.
  • Plan and execute community education, screening, and health-literacy events; own logistics, staffing, materials, patient intake, follow-up, and conversion of identified needs into scheduled care.
  • Maintain audit-ready documentation, monitor allowable costs and time-and-effort requirements, prepare grant deliverables and progress reports, and coordinate compliance with HIPAA, OSHA, infection control, telehealth, licensure, and documentation standards.
  • Define, validate, and report the metrics that matter — patients served by county, new patient volume, wait times, no-shows, schedule utilization, referral conversion, service-line use, screening yield, follow-up completion, and documented access barriers.
  • Track budget performance, volume, payer mix, cost per site day, and contribution; flag variances early and recommend changes to staffing, site cadence, workflow, or service mix that improve sustainability without disrupting hub operations.
  • Manage contracts and performance for space, transport, equipment, supplies, imaging, connectivity, and other rural operations partners; keep renewal dates, costs, responsibilities, and dependencies visible.
  • Find the root cause of recurring failures, redesign the process, document it in clear SOPs and checklists, train the team, and audit whether the fix is working across every site.
  • Serve as a credible, responsive on-the-ground representative with referral partners, patients, community groups, vendors, and local stakeholders; travel regularly to rural sites and outreach events to lead firsthand.

Required Skills/Abilities

  • Builder’s mindset: proven ability to take a site, service line, or program from concept to go-live and then operate it with discipline.
  • High ownership and sound judgment: identifies the problem, owns the next step, and arrives with a safe, compliant, cost-effective recommendation.
  • Strong foundation in outpatient practice operations, staffing, scheduling, patient flow, revenue-cycle basics, credentialing, payer enrollment, and healthcare compliance.
  • Program and project management skills sufficient to run multiple site launches, workstreams, and event calendars without dropped commitments.
  • Data discipline and financial fluency: can define a metric, validate the data, read a budget, explain variances, and translate findings into action and dollars.
  • Confident, concise communication with executives, providers, staff, funders, referral partners, community organizations, vendors, and local officials.
  • Technology fluency, including eClinicalWorks or a comparable EHR, advanced spreadsheets, dashboards, and practical use of AI-assisted workflows with appropriate verification.
  • Cultural competence and ease working in rural communities, including with patients and partners facing transportation barriers, limited health literacy, or long-standing distrust of the healthcare system.
  • Valid Florida driver’s license, reliable transportation, and willingness to travel regularly to rural sites and outreach events.

Education and Experience

  • Bachelor’s degree required; substantial relevant healthcare operations experience may be considered in lieu of a degree. Master’s degree in health administration, public health, or business preferred.
  • At least five years of healthcare operations, practice management, or program administration experience with direct accountability for outcomes.
  • Experience opening a new healthcare site, service line, or program from the ground up strongly preferred.
  • Experience administering grant-funded programs, including budget tracking, time-and-effort documentation, and funder reporting, strongly preferred.
  • Experience in rural health, community health, an FQHC, a rural health clinic, or a critical access setting strongly preferred.
  • Working familiarity with credentialing, payer enrollment, and multi-location place-of-service requirements.

Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer.
  • Regular travel between the Jacksonville area and rural clinic sites, including some early starts and occasional evening or weekend outreach events.
  • Ability to lift up to 25 pounds and to load, transport, and set up equipment, supplies, and event materials for offsite clinics and community events.
  • Ability to work productively in smaller rural facilities, construction or buildout environments, and temporary event spaces with limited support staff on site.
