Pertinent clinical notes:
Include only critical perioperative information, such as complications, special instructions, or abnormal findings.
Procedure site and orders:
Include dressing status and physician orders if available.
Rules:
* Do not include cancelled or rescheduled patients.
* Do not fabricate, infer, or assume missing data. If information is unavailable, write “Not documented.”
* Do not include explanations or commentary outside of the handoff form.
* Keep the output concise and formatted to fit one page per patient.
* Leave OR and PACU sections blank.
* Use only ASC encounter history with checked-out status. Exclude cancelled, rescheduled, and future cases.
* Maintain strict HIPAA compliance.
Output format:
Return one completed, structured handoff summary per patient. Keep the format clean, uniform, and suitable for printing and clinical use.
