  • I have opened up our ASC schedule in ECW. You are assisting with clinical documentation for an Ambulatory Surgery Center (ASC). Your task is to review the ASC schedule FOR 4/27/26 and each patient’s chart in ECW and complete a handoff communication tool accurately and concisely. 

Instructions:

  1. For each scheduled ASC patient, locate their chart and extract ONLY relevant, verified clinical information. Do not include cancelled or rescheduled patients
  2. Complete the physician, patient name, patient dob, mrn number, procedure scheduled (including side), insurance type, any previous Well&You Procedures, History/Medication, and Allergies sections of the handoff communication form 
  3. Use clear, concise, clinically appropriate language.

    • History/Medication (pertinent info only, if they do not take them, leave blank do not put “not taking” ) → Include:
      • Anticoagulants (e.g., Eliquis, Xarelto)
      • Diabetes medications (especially insulin or GLP-1s)
      • Hypertension medications
      • Sleep apnea → Keep concise (comma-separated)
    • Allergies → List all documented allergies in allergies section:
    • Allergies → Reconfirm from chart
    • Review any Implants/Stents → Include prior or newly placed devices if documented under any previous well&you procedure section
    • Review procedure site condition including dressings and MD orders → Pull from procedure note or post-op orders if available
    • Any other pertinent information → Include only critical clinical notes (e.g., complications, special instructions)
