Heart disease is the leading cause of death for women in the United States, and only about 56% of women know it (CDC). That awareness gap has consequences: women are diagnosed later, treated less aggressively, and more likely to have symptoms attributed to anxiety, reflux, or stress before a cardiac cause is considered.

The Numbers

In 2023, heart disease was responsible for 304,970 deaths among US women — about 1 in every 5 female deaths (CDC Heart Disease Facts). More than 58.9 million US women (45.7%) have high blood pressure or take medication for it. After menopause, coronary heart disease risk rises as estrogen’s protective vascular effects diminish.

Why Heart Disease Presents Differently in Women

The classic picture — crushing central chest pain radiating down the left arm — is drawn from studies conducted predominantly in men. Women frequently do experience chest pain, but they are considerably more likely than men to present with symptoms that are not recognized as cardiac:

  • Unusual, profound fatigue, sometimes for days or weeks beforehand
  • Shortness of breath with ordinary exertion that was previously easy
  • Pain in the jaw, neck, upper back, or between the shoulder blades
  • Nausea, vomiting, or indigestion-like discomfort
  • Cold sweats, lightheadedness, or unexplained anxiety
  • Sleep disturbance in the weeks before an event

Women are also more likely to have coronary microvascular disease and INOCA — ischemia with no obstructive coronary arteries. In these conditions the small vessels of the heart malfunction while a standard angiogram looks clean. Being told “your arteries are fine” while symptoms persist is a common and frustrating experience, and it does not mean nothing is wrong.

Risk Factors Specific to Women

Beyond the shared risks of hypertension, high cholesterol, diabetes, smoking, obesity, inactivity, and family history, women carry risk factors that are routinely omitted from standard cardiac risk calculators:

  • Preeclampsia or gestational hypertension — roughly doubles lifetime cardiovascular risk
  • Gestational diabetes — elevated risk independent of later type 2 diabetes
  • Preterm delivery and recurrent pregnancy loss
  • Polycystic ovary syndrome (PCOS)
  • Early menopause, before age 45, natural or surgical
  • Autoimmune disease — lupus and rheumatoid arthritis substantially raise cardiovascular risk
  • Breast cancer treatment history — certain chemotherapies and chest radiation carry cardiotoxicity
  • Migraine with aura

If you have any of these, your true risk is likely higher than a standard calculator reports. Bring them up — they belong in the history even decades later.

What a Women’s Heart Evaluation at Well&You Includes

  • Comprehensive risk assessment including the pregnancy, menopause, and autoimmune history above
  • Blood pressure evaluation, with ambulatory monitoring where clinic readings are inconsistent
  • Advanced lipid panel including lipoprotein(a), which is genetically determined, underused as a test, and disproportionately informative in women
  • Metabolic screening — A1C, fasting glucose, inflammatory markers
  • ECG and echocardiography
  • Coronary calcium scoring (CT) — a direct, low-radiation measure of existing plaque burden. Dr. Shah’s board certification in cardiac CT is directly relevant here
  • Stress testing when indicated
  • Vascular screening — carotid duplex and ankle-brachial index, since peripheral artery disease frequently coexists with and signals coronary disease

Who Should Be Evaluated

Schedule an evaluation if you are over 40 and have never had a cardiovascular risk assessment; have any pregnancy complication in your history; have entered menopause; have a family history of early heart disease (father or brother before 55, mother or sister before 65); have hypertension, diabetes, or elevated cholesterol; have an autoimmune condition; have completed breast cancer treatment; or have symptoms that have been dismissed and remain unexplained.

Call 911 immediately for chest pain or pressure lasting more than a few minutes, pain spreading to the jaw, neck, back, or arm, sudden shortness of breath, or cold sweats with nausea and lightheadedness. Do not drive yourself.

Prevention, Recovery and Ongoing Care

For women who have already had a cardiac event or procedure, recovery is structured: cardiac rehabilitation, medication optimization, and scheduled follow-up. Women are referred to and complete cardiac rehab at consistently lower rates than men despite equal or greater benefit, so we treat that referral as part of the visit rather than an afterthought.

For everyone else, prevention is where the outcome is decided. Blood pressure control to target, lipid management including statin therapy when indicated, glycemic control, structured exercise, smoking cessation with real support rather than advice, sleep apnea screening — a frequently missed contributor in women — and stress and mental health support, given the documented cardiovascular impact of chronic stress and depression.
